Volunteer Insurance Reporting Form

This form must be completed by each county prior to May 31st of each year.

Volunteer Insurance

Volunteers performing duties according to their volunteer job descriptions are covered under an annual volunteer insurance liability policy. This is personal, secondary liability insurance of up to $1,000,000 per occurrence. This does not cover transportation liability and volunteers are encouraged to seek insurance advice from their insurance agent when transporting individuals. This does not cover accident insurance or medical or health coverage and does not cover negligence or errors on the part of the volunteer.

The policy runs from July to June of each year and is paid in full by Extension Administration (starting in 2016) and costs around $50,000 annually.

Invoices should not be paid from your local checking account or by affiliated committees and groups.

All that is required is that you complete the form below by May 31st of each year.
